This website provides general information about Mountain Community Healthcare. It does not directly accept medical documents, patient messages, or online contact form submissions. Membership enrollment is provided through an embedded third-party AtlasMD form.

For medical questions, records, and secure patient communications, use the clinic-approved AtlasMD workflow after your account is activated. The membership enrollment form is hosted by AtlasMD and subject to its applicable practices and notices.

Do not submit sensitive medical information through ordinary email or other unverified channels. For a copy of the practice’s official HIPAA Notice of Privacy Practices, please contact the office.

This website privacy information is separate from the practice’s HIPAA Notice of Privacy Practices.
