Complete the enrollment form below. The form is provided and processed by AtlasMD, the practice’s patient management system. Mountain Community Healthcare will review your enrollment and provide instructions for accessing the patient app after approval.
Before you begin: Review the membership details and pricing. You can also call 423-202-3036 for help enrolling. After enrollment, the clinic will send you a secure patient intake form and instructions for scheduling your first visit. If the form does not appear on your device, use the direct enrollment link.

Interested in membership for your team? Contact the clinic at the office through our Contact page for general program information. A separate AtlasMD corporate enrollment form will be added once the practice provides and approves it.
